We present the case of a 53-year-old woman with headaches and new onset left sided hemiparesis and paresthesia. Imaging revealed platybasia and abnormal ligament calcification at the craniocervical junction causing spinal cord compression. Surgical intervention involved a two-stage procedure: an endoscopic endonasal approach for anterior C1 arch removal, odontoidectomy, and inferior clivusectomy, followed by occiput to C4 posterior fusion. Post-operative imaging confirmed successful decompression at this level. The patient showed significant improvement at 6-week follow-up.
