In this surgical video we demonstrate the surgical steps and relevant anatomy of a pre-auricular subtemporal approach to a middle fossa paraganglioma with infratemporal extension. A frontotemporal scalp flap with preauricular extension was elevated and temporalis retracted forward. The mass was seen invading through bone and into the temporalis. A craniotomy was performed encircling the invading mass. Microdissection along the MMA feeders disconnects the tumor, followed by piecemeal debulking. A multilayered closure ensues.
