V069: ENDOSCOPIC ENDONASAL TRANSTUBERCULUM RESECTION OF SUPRAGLANDULAR RATHKE'S CLEFT CYST WITH COMPLETE STALK INVOLVEMENT: TECHNICAL NUANCES

The nuances of a safe dissection of an endoscopic endonasal transtuberculum supraglandular Rathke’s cleft cyst with complete pituitary stalk involvement are discussed. After exposing the tumor in the suprasellar cistern, internal debulking was followed by sharp dissection of the superior hypophyseal artery from the tumor capsule. Then the non-viable stalk, which was completely involved by the tumor, was sharply transected, given preoperative panhypopituitarism to achieve gross total resection. Countertraction and cross-suction techniques were minimally used.
