A 6-year-old boy presented with progressive visual decline and behavioural changes. Pre-operative MRI demonstrated a large cystic sellar and suprasellar lesion compressing the optic chiasm, consistent with craniopharyngioma. An image-guided Ommaya reservoir was inserted for cyst decompression and optic apparatus relief. Definitive resection was performed two months later via an expanded endoscopic endonasal approach, achieving near-total resection. Postoperatively, vision improved, and the patient developed new diabetes insipidus following planned pituitary stalk sacrifice.
