We present a case of a complex recurrent multiple CSF leaks in a 3-year-old girl with a background of severe respiratory disease. Three approaches over the course of two years were undertaken to repair the leak, predominantly from the right SOF and foramen rotundum. First, this was an EEEA, followed by a lateral orbitotomy, and finally a right endoscopic TPF flap.
