V058: LSR-GUIDED MICROVASCULAR DECOMPRESSION FOR HEMIFACIAL SPASM

Microvascular decompression (MVD) is considered the gold standard for treatment of hemifacial spasm (HFS) caused commonly by facial nerve (CN VII) compression. Lateral spread response (LSR) provides real-time monitoring of CN VII decompression. We present two cases demonstrating the role of LSR during intraoperative decision-making. In one, persistent LSR prompted further exploration and decompression of a second offending artery, abolishing LSR. In the other, complete LSR resolution averted riskier microdissection of a second intermingled vessel.
