A 27 year old female presented with headaches and was found to have a large ventral cervicomedullary junction tumor. We performed an endoscopic transclival tumor resection, using 70° endoscope and angled instruments, which allowed enhanced visualization and precise dissection of the inferior margin without disruption of craniocervical junction stability. The pathology of pilocytic astrocytoma is also quite uncommon in this location. This approach demonstrates the versatility of angled endoscopic minimally invasive techniques.
