We present the case of a 70-year-old male with progressive right-sided V1–V3 neuropathy, including hemifacial pain and numbness. Imaging demonstrated a Meckel’s cave mass involving V3 with extension through the foramen ovale, suspicious for a trigeminal schwannoma (TS). Resection via an endoscopic transorbital approach (ETOA) achieved gross total resection without complications. The patient’s pain resolved completely. No recurrence observed at 2.5 years. This case highlights the utility of ETOA in managing selected TS cases.
