This video demonstrates endoscopic transnasal transclival resection of a ventral foramen magnum meningioma. After presenting clinical and pre-operative imaging, step-by-step narration illustrates exposure, dissection, and safe brainstem decompression. Post-operative imaging confirms resection. Compared with the far lateral approach, the endonasal route provides a direct midline corridor while avoiding brainstem and cranial nerve retraction. The video underscores technical nuances, safety strategies, and highlights endonasal surgery as a viable alternative for challenging skull base pathology.
