Marco Antonio Munuzuri-Camacho, MD1; Maria Fernanda Tejeda-Pineda, MD2; Luis Alberto Ortega-Porcayo, MD3; Marcos V. Sangrador-Deitos, MD4; Victor Alcocer-Barradas, MD1; 1Neurosurgery Department, National Institute of Neurology and Neurosurgery, Mexico City, Mexico; 2Neurosurgery Department, Baylor College of Medicine, Texas, USA; 3Neurosurgery Department, Hospital Angeles Pedregal, Mexico City, Mexico; 4Neurosurgery Department, Hospital Zambrano Hellion, Monterrey, Mexico

This video demonstrates endoscopic transnasal transclival resection of a ventral foramen magnum meningioma. After presenting clinical and pre-operative imaging, step-by-step narration illustrates exposure, dissection, and safe brainstem decompression. Post-operative imaging confirms resection. Compared with the far lateral approach, the endonasal route provides a direct midline corridor while avoiding brainstem and cranial nerve retraction. The video underscores technical nuances, safety strategies, and highlights endonasal surgery as a viable alternative for challenging skull base pathology.
