We present a 34-year-old female with progressive visual loss caused by a tuberculum sellae meningioma invading the left optic canal. Preoperative MRI demonstrated significant compression of the optic nerve. The patient underwent an endoscopic endonasal approach with bilateral sphenoidotomy, tumor debulking, and meticulous extracapsular dissection. Gross total resection was achieved with decompression of the optic apparatus. This video illustrates key anatomical landmarks, surgical steps, and reconstruction techniques for safe management of such challenging lesions
