Large Trigeminal Schwannomas can occupy multiple compartments requiring extended skullbase approaches. Here we present a case of a 14-year-old female patient with a progressively enlarging trigeminal schwannoma with brainstem compression.  We demonstrate the techniques and nuance of a middle fossa approach in addition to dissection methods that we propose obviate neurological injury and cerebrospinal fluid leak.  This allows single stage surgery. Gross total resection was achieved and the patient remained well.
