This video abstract presents the use of a combined open supraorbital craniotomy and endoscopic endonasal approach for resection of a left sinonasal/orbital osteoma with skull base extension and mass effect of the patient’s extraocular muscles and optic nerve. While the lesion’s involvement of the frontoethmoid sinuses are endoscopically accessible, a combined open and endoscopic approach was chosen to allow for better visualization and to minimize manipulation of orbital structures.
