V051: RETROSIGMOID RESECTION OF A GIANT LEFT VESTIBULAR SCHWANNOMA IN THE SITTING POSITION

This video presents the microsurgical resection of a giant left vestibular schwannoma in a 19-year-old woman with Turner syndrome. A gravity-assisted retrosigmoid approach in the sitting position was used. Near-total resection was achieved using bimanual dissection under continuous neurophysiological monitoring. A high-riding jugular bulb precluded drilling of the internal auditory canal, leaving minimal intracanalicular tumor. Postoperative imaging confirmed brainstem decompression. The patient developed a transient House–Brackmann grade II facial palsy, which fully resolved.
