We present the case of a 66-year-old woman with an incidentally discovered anterior cranial fossa floor ethmoidal dural arteriovenous fistula (dAVF). Ethmoidal dAVFs are rare vascular lesions, which may be associated with a higher incidence of hemorrhage than dAVFs located elsewhere. After discussion of the risks and benefits of observation, endovascular treatment, or open surgery, the patient elected to proceed with craniotomy. The fistula was successfully ligated via a supraorbital approach with endoscopic assistance.
