V045: CLIPPING OF GIANT ICA ANEURYSM AND DECOMPRESSION OF BILATERAL OPTIC NERVES VIA ORBITO-PTERIONAL AND ANTERIOR CLINOIDECTOMY: 2D MICROSURGICAL VIDEO

Giant intracranial aneurysms may cause cranial neuropathy via direct compression, or flow related pulsatility. In these instances open decompression and occlusion may be required. We present a case of a giant internal carotid artery (ICA) aneurysm causing bilateral optic neuropathy. The patient underwent orbito-pterional craniotomy, extradural anterior clinoidectomy, and clip reconstruction of the ICA and occlusion of the aneurysm. This video highlights the technical steps and nuances required. The patient and their vision recovered well.
