A 31-year-old woman presented with a second bleed from her tectal large cavernoma, resulting in bilateral CN IV palsies and upgaze palsy. A supracerebellar infratentorial approach in sitting position with division of the tentorium for additional supratentorial access was utilized to resect the cavernoma. No intraoperative complications were incurred and the patient was discharged home on post operative day 3 and was able to attend her own wedding four weeks later.
