V041: FAR LATERAL CRANIOTOMY WITH CONDYLAR FOSSA DRILLING FOR CLIPPING OF RUPTURED PROXIMAL POSTERIOR INFERIOR CEREBELLAR ARTERY ANEURYSM

Proximal posterior inferior cerebellar artery (PICA) aneurysms exhibit complex morphology, limiting endovascular treatment indications; microsurgical clipping is sometimes crucial. Far lateral craniotomy with condylar fossa drilling and harvesting the occipital artery (OA) for OA–PICA anastomosis is considered effective. We report a case of a ruptured proximal PICA aneurysm successfully treated using this strategy. As endovascular techniques expand, maintaining microsurgical proficiency is essential for effective management of proximal PICA aneurysms.
