A 58-year-old woman presented with headache and nuchal rigidity but was otherwise neurologically intact, was found to have HH2F3 SAH due to ruptured left fetal PCOM aneurysm. A left cranio-orbital approach with anterior clinoidectomy permitted successful clipping of the aneurysm. Postoperative DSA confirmed that the left anterior choroidal artery and fetal PCOM remained patent and that the aneurysm was obliterated. She recovered well and is functionally independent at three-month follow-up.
