A 55-year-old female presented with progressive vision loss leading to complete blindness in the left eye, secondary to a spheno-orbital meningioma. She underwent pterional craniotomy, followed by gamma knife radiosurgery for three additional meningiomas. The spheno-orbital residual continued to progress despite additional stereotactic radiosurgery, ultimately necessitating expanded endonasal resection. The patient tolerated the procedure well with no neurological deficits.
