A 45-year-old female without past medical history presented with headaches, occasional blurry vision, and focal seizures. Imaging revealed a 3.2 cm left medial sphenoid wing meningioma with invasion of the optic nerve sheath and displacement of the left optic nerve and internal carotid artery. She underwent a modified left mini pterional approach with extradural clinoidectomy for tumor resection. She remained neurologically intact postoperatively and there was no evidence of residual on 1-year follow up imaging.
