A 63-year-old woman presented with a slowly enlarging right sphenoid wing meningioma. A transorbital neuroendoscopic approach (TONES) via an eyelid crease incision with orbital rim reduction was chosen. For well selected lesions, the TONES technique enables direct access to the middle cranial fossa with minimal brain retraction and good cosmetic outcomes. With orbital rim reduction, need for lateral orbitotomy may be avoided; however, the orbital contents must be handled carefully to avoid associated visual complications.
