This video demonstrates an extended trans-orbital approach for the resection of a right spheno-orbital meningioma. We utilized a modified lateral orbitotomy leaving the orbital rim in place connected to the temporalis muscle with osteotomies facilitating mobilization of the segment to achieve the desired lateral to medial operative trajectories. This approach did not compromise the operative corridor or require a compensatory increase in globe retraction. Reconstruction was simplified with improved cosmesis and decreased morbidity.
