Temporal bone meningiomas are rare entities and present a formidable surgical challenge for complete resection. In this operative video, we demonstrate a combined skull base approach incorporating both middle fossa and translabyrinthine corridors for resection of a large temporal bone meningioma with facial nerve involvement. A sural nerve graft was utilized for reanimation via interposition grafting. This video highlights key anatomical landmarks, microsurgical techniques, and decision-making strategies that underpin successful execution of this complex procedure.
