Expanded endoscopic endonasal approaches (EEAs) enable direct access to petroclival chondrosarcomas while avoiding cerebellar retraction and cranial nerve dissection required in lateral approaches. These tumors frequently infiltrate the sphenoid and clival bone, necessitating wide bony removal that is prerequisite for EEA. We report a 69-year-old female with progressive imbalance, diplopia, facial weakness, and hearing loss, who successfully underwent EEA with a contralateral transmaxillary corridor for substantial tumor debulking and brainstem decompression.
