We present a 41-year-old male with right-sided 6th nerve palsy who underwent an endoscopic endonasal extended transsphenoidal, transpterygoid-sublacerum approach for resection of a right petroclival chondrosarcoma. The tumor involved the petrous, lacerum, and paraclival segments of the ICA, Meckel’s cave, and Dorello’s canal. Adequate resection was achieved while preserving the abducens nerve and vascular structures. Reconstruction employed an abdominal fat graft and nasoseptal flap. No complications occurred.
