A 33-year-old woman presented with right VI nerve paresis with imaging revealing a calcified tumor in the petrous apex region arising from upper petroclival fissure with invasion of inferior compartment of the cavernous sinus and displacement of the internal carotid artery consistent with a chondrosarcoma. Endoscopic endonasal transpterygoid transclival approach with interdual pituitary hemitransposition was used to achieve gross total resection. Postoperatively there was worsening of the abducens nerve function and transient diabetes insipidus.
