V023: COMBINED TRANSCLIVAL-TRANSTUBERCULUM ENDOSCOPIC ENDONASAL APPROACH FOR A SUPRASELLAR AND POSTERIOR FOSSA EPIDERMOID CYST RESECTION

Intracranial epidermoid cysts are rare tumors that may cause cranial nerve or brainstem compression. We present a 66-year-old woman with imbalance, facial numbness, and headaches caused by a suprasellar–posterior fossa epidermoid cyst. An endoscopic endonasal dual trans-clival and trans-tuberculum approach enabled safe resection of the mass. Postoperatively, patient developed transient sixth nerve palsy and permanent fourth nerve palsy accommodated with prisms. This technique highlights the utility of expanded endonasal corridors for expansile midline lesions.
