V021: EXTREME-MEDIAL APPROACH WITH EUSTACHIAN TUBE MOBILIZATION FOR RECURRENT CLIVAL CHORDOMA

We report a recurrent clival chordoma involving the lower clivus, hypoglossal canal, jugular tubercle, and occipital condyle. Using an extreme-medial endonasal transpterygoid approach with Eustachian tube mobilization, we achieved safe condylar drilling, full exposure of the hypoglossal nerve, and gross total resection. Reconstruction was performed with fat and mucosal grafts, followed by planned occipitocervical fusion.
