Persistent craniopharyngeal canal defects (PCCD) with encephalocele are a rare but well described skull base pathology; this is the first surgical video illustrating a repair to our knowledge. The patient is a 27-year-old female with amenorrhea and visual deficits in the setting of a PCCD with encephalocele. She was taken for an endoscopic endonasal repair during which the encephalocele was reduced into the sella and the PCCD was repaired without CSF leak.
