This case highlights the technical nuances and endoscopic anatomy associated with the posterior compartment of the cavernous sinus. We present a 51-year-old female with a non-functioning pituitary adenoma, previously resected at an outside institution. She subsequently presented with persistent headaches, and surveillance imaging revealed progressive growth of residual tumor involving the left cavernous sinus.
