This is a case of a 23 year old male with acromegaly who underwent an endoscopic endonasal approach for pituitary tumor removal with thickened medial cavernous sinus wall resection. Meticulous and safe tumor dissection was performed. Gross total resection was achieved as well as biochemical remission with resulting clinical improvement.
