We describe the case of a 23-year-old male who presented with obstructive hydrocephalus secondary to a large multi-cisternal epidermoid, primarily centered in the supra-sellar cistern with extension into the retro-clival area. The patient underwent a ventriculostomy followed by expanded endoscopic endonasal approach (EEA) for maximal safe resection of this lesion. Post-operatively, the patient did well neurologically however required a VP shunt for hydrocephalus.
