We present the endoscopic endonasal resection of a giant tuberoinfundibular craniopharyngioma in a 10-year-old boy. Our technique involved hypothalamic subpial dissection to maximize tumor removal while preserving hypothalamic function. Postoperative imaging confirmed gross total resection, recovery was uneventful, with no recurrence at 10 months follow-up. Postoperatively comprehensive neurocognitive and behavioral evaluation showed all domains performing at or above population averages.
