The technical nuances of a pseudocapsular technique for resection of a large pituitary macroadenoma are discussed. The goal was to preserve the continuity of the gland while achieving maximal resection. Angled curettes and suction were used in the inferior aspect of the tumor to first demarcate the gland-tumor interface. Using sharp dissection for adherent part of the interface, the plane was advanced from posterior to superior aspect to finally free the tumor from the diaphragm.
