V012: ENDOSCOPIC ENDONASAL RESECTION OF PURELY INTRAVENTRICULAR CRANIOPHARYNGIOMA

Purely intraventricular craniopharyngiomas (Kassam type IV) are regarded as inaccessible via an endonasal approach due to risk of hypothalamic injury. We present a 61-year-old female with hypopituitarism, including diabetes insipidus and anterior pituitary dysfunction and a cystic–solid suprasellar mass arising within the third ventricle and above the infundibulum. Expanded endoscopic endonasal transtuberculum resection achieved near-total removal, sparing tumor adherent to the hypothalamus/optic tracts with planned radiosurgery to the adherent residual.
