A 52-year-old male presented with several months of altered mental status, confusion, and memory impairment. Imaging demonstrated a tuberoinfundibular craniopharyngioma with significant extension into the third and lateral ventricles, leading to pituitary dysfunction and cognitive decline. A staged approach was considered in the event of incomplete resection. The patient underwent expanded endoscopic endonasal resection with planned pituitary sacrifice. Two small residual foci of enhancement adherent to the bilateral hypothalami were planned for subsequent radiosurgery treatment.
