We present a 48-year-old male with a progressive cavernous sinus lesion causing ophthalmoplegia after failed radiosurgery. Imaging showed internal carotid artery (ICA) encasement. The patient underwent cavernous sinus tumor resection with ICA sacrifice and EC-IC bypass using a radial artery graft. Postoperative imaging confirmed gross total resection without complications or new deficits. Final pathology revealed a solitary fibrous tumor.
