V005: ECA-RA-P2?PCA BYPASS COMBINED WITH PROXIMAL ANEURYSM OCCLUSION FOR THE TREATMENT OF VBD: A CASE REPORT

A 45-year-old man with VBD underwent left ECA–RA–P2 PCA bypass. Intraoperative ICG and immediate postoperative DSA confirmed graft patency. One month later, proximal occlusion of the VBD was performed; the bypass remained patent. At six months, DSA showed disappearance of the aneurysm with retrograde flow to the VBJ. Long-term follow-up demonstrated stable aneurysm size, improved perfusion, and clinical recovery, with mRS improving from 3 to 2 at 63 months.
