P489: POSTERIOR PONTINE CAVERNOMA. TELOVELAR/ SUPRACOLLICULAR APPROACH

This video presents the microsurgical resection of a hemorrhagic posterior pontine cavernous malformation via a unilateral telovelar approach using a supracollicular safe entry zone. Preoperative imaging showed inferolateral displacement of the facial colliculus. Gravity-assisted bimanual dissection and intraoperative neurophysiological monitoring enabled en bloc resection while preserving surrounding structures. Postoperative MRI confirmed complete resection without complications. The patient developed a transient House–Brackmann grade IV facial palsy, which improved to grade III at follow-up.
