P432: LEFT FAR LATERAL TRANSCONDYLAR APPROACH FOR RESECTION OF A VENTRAL FORAMEN MAGNUM MENINGIOMA

We present a complex case of a ventral foramen magnum meningioma resected via a left far lateral approach. The patient had progressive tumor growth with severe brainstem compression but minimal symptoms aside from neck pain. Surgical challenges included a fibrotic, firm tumor, encasement of the vertebral artery at its dural entry point, and a rare variant with a proximally arising PICA draped over the tumor. Near-total resection was achieved with complete neurovascular preservation.
