P422: NUANCES ON JUDICIOUS USE OF NASO-SEPTAL FLAP IN EXTENDED ENDONASAL APPROACHES FOR REMOVAL OF PITUITARY MACROADENOMA WITH DIFFERENT PATTERNS OF SUPRASELLAR EXTENSION

Using nasoseptal flap(NSF) or not, for sellar reconstruction in extended endoscopic endonasal approaches for macroadenomas depends on pattern of suprasellar extension. Case 1- NSF was used where adenoma had horizontal extension over the planum sphenoidale and diapgragmatic defect (DD)>1cm. Case 2: Adenoma with no planum extension however was compressing the third ventricle floor-ACA complex where the DD<1cm; dual suction technique was used for intra-arachnoidal extracaopsular resection and was followed by multilayer modified reconstruction without NSF.
