P421: NUANCES OF THE ENDOSCOPIC ENDONASAL TRANSCAVERNOUS RESECTION OF A RECURRENT AND REFRACTORY KNOSP 4 MACROPROLACTINOMA

Step-by-step nuances of the endoscopic endonasal medial transcavernous approach for a 23 y/o male who presented with a recurrent medically refractory Knosp 4 prolactinoma with right cavernous sinus invasion resistant to medication are discussed. Complete resection of the medial, posterior, and superior compartments, along with a significant decrease in prolactin level, was achieved postoperatively. There was a small residual tumor in the lateral compartment which could benefit from adjuvant radiotherapy.
