A 76-year-old male with a 14-month history of left retro-orbital headaches was found on imaging to have a left sphenoid mass with skull base erosion. CT revealed a heterogeneous anterior sphenoid lesion and a posterior fungal ball with associated mucocele. Endoscopic sinus surgery was performed to clear the amorphous material and purulence from the sinus, and to drain the mucocele without CSF leak. Postoperatively, the patient experienced complete resolution of headaches.
