P413: MACROVASCULAR DECOMPRESSION OF TRIGEMINAL NERVE FOR DOLICHOECTATIC BASILAR ARTERY WITH SLING TECHNIQUE

Trigeminal neuralgia (TN) is a chronic facial pain disorder marked by electric shock-like pain. Rarely, a dolichoectatic vertebrobasilar artery compresses the trigeminal nerve root entry zone, causing refractory TN. Macrovascular decompression with sling transposition is a key surgical approach. We present a 63-year-old man with 4 years of progressive right-sided facial pain. This report reviews clinical findings, operative technique, and offers technical pearls for successful decompression.
