This is a 62-year-old man presenting with seizures and brain congestion due to a large right parasagittal venous aneurysm secondary to an AVF involving several bilateral feeder arteries (STA, MMA, occipital). A bicoronal craniotomy was performed to clip the inlet vein of the fistula. Heavy vascularization of the peri-aneurysmal skull and scalp required pre-emptive suturing, temporary clipping of the STA, and diamond drill-guided craniotomy. The patient has since made a full neurological recovery.
