P405: FAR LATERAL TRANSCONDYLAR CRANIOTOMY FOR LEFT PROXIMAL PICA ANEURYSM - COTTON CLIPPING TECHNIQUE
Paul Marcet, MD; Allison Medina, MD; Omaditya Khanna, MD; John Wilson, MD, FAANS, FACS; Wake Forest

In this video we demonstrate the cotton clipping technique of a ruptured left posterior inferior cerebellar artery aneurysm via the far lateral transcondylar approach. The patient presented with a 2mm aneurysm arising proximally from the lateral medullary segment of PICA. After opening the dura there was intraoperative aneurysmal rupture, which was carefully controlled with bipolar cautery to the aneurysm, and the aneurysm was secured with a muslin gauze wrap around PICA to include the aneurysm.
