P404: MULTIPORTAL APPROACH TO A LARGE SINO-CRANIO-ORBITAL GIANT SKULL BASE OSTEOMA

This video illustrates the management of a giant frontal sinus osteoma in a 70-year-old male involving the orbit, ethmoid sinus, and skull base. A combined open osteoplastic flap, endoscopic endonasal, and transorbital approach enabled complete tumor resection. Intraoperative management included cerebrospinal fluid leak, successfully managed with multilayer reconstruction using free mucosal graft. Postoperative imaging confirmed gross total resection. This case underscores the value of multidisciplinary collaboration and multimodal, multi-portal approaches to skull base pathology.
