This video demonstrates a patient with nasal obstruction, facial deformity, epiphora, and a missing canine who had a large left expansile sinonasal lesion associated with an ectopic tooth. Exclusively endoscopic resection was performed using Denker’s modification of medial maxillectomy with stereotactic guidance. Endoscopic sinus surgery and a dacryocystorhinostomy were performed to manage their obstructive sinusitis and epiphora. Gross total resection was achieved with no post-operative complications. This approach enabled complete endoscopic resection with minimal morbidity.
