We present the case of a 58-year-old male who presented with a large, 2.6 x 2.2 x 3.0 cm pituitary macroadenoma invading the left cavernous sinus and compressing the optic chiasm. Traditionally, reliance on tumor descent may lead to residual tumor hiding within the redundant diaphragma sellae. To prevent collapse of the surgical cavity walls in large adenomas, classic cerebrospinal fluid diversion techniques such as mannitol can maximize endoscopic visualization of the surgical cavity.
