A 71 year old gentleman presented for a 4 months history of headaches and pressure, and 1 week history of diplopia and decreased visual acuity. Imaging showed bony erosion of the sphenoid sinus along with a heterogenous density, and inflammatory changes in the orbital apex and pterygopalatine fossa. A biopsy of the sphenoid sinus was consistent with fungal ball. He worsened postoperatively and managed with a pterygopalatine fossa exploration, inferior orbital fissure biopsy and debridement.
